Ordering a Birth Certificate
Effective February 1st, 2016 Berkeley Vital Records will only issue Certificates for the first 5 years after the Birth. After 5 years of birth, the certificate can be obtained by contacting the Alameda County Recorder's Office, (510) 272-6362
OBTAINING A BIRTH RECORD
The City of Berkeley Vital Records maintains birth certificates for babies born within Berkeley City limits only. These birth certificates can be purchased from our office until the child's first 5 years. Once the child is 6 years old the birth certificate can be obtained by contacting the Alameda County Recorders Office, (510) 272-6362. If a birth occurred outside Berkeley, contact the county, state, or country in which the event occurred to request the record. City of Berkeley does not issue marriage certificates. Contact the Alameda County Recorder at (510) 272-6363 for marriages in Berkeley.
The California Health and Safety Code, Section 103526, permits only authorized persons as listed to receive certified copies of birth or death records. Requests submitted by mail or by fax must include a notarized statement attesting to the identity of the requestor attached to the application. Individuals requesting in person can receive an authorized copy after first submitting a sworn statement under penalty of perjury that the requester is an authorized person.
Effective July 1st, 2015, Assembly Bill 1733 states that each local registrar or county recorder is mandated to issue a birth record without fee to an eligible homeless person, or homeless child or youth. Eligible persons must present an affidavit signed by an agent for a homeless services provider to receive a fee-exempt copy. Completed affidavits should be taken at face value and processed the same as sworn statements when applicants sign under penalty of perjury. County vital records offices are not responsible for verifying if a homeless services provider is legitimate.
Certified certificates are issued on a long form (8 1/2 " x 11") security paper with an official embossed seal, acceptable for passport, social security, employment, and personal identification purposes and other legal purposes. Detailed instructions are given below.
- Applicant must show a VALID Picture ID
- Full name of the child/person as it appears on the Birth Certificate
- Date of birth
- Mother's full maiden name
Also useful but not required:
- Father's full name
- Hospital/place of birth
