Online Forms 

The Berkeley Police Department continues to add online forms to our Web site in an effort to make it easier for members of the community to access police services. Any information provided is used solely to assist the Police Department in responding to the needs of the community.

• Traffic Enforcement Form Use this form to report specific traffic related problems in your neighborhood

• Business Emergency Contact Form This form allows business owners to provide emergency responders
  with after hours contact information

• The Disabled Registry Form provides first responders with information about specific needs of disabled residents and will help police and fire get specific resources to assist them in an emergency response or evacuation.

• No Trespass (person)  This form is used when you have a named person that you want to keep off your property.

• No Trespass (location)  This form is used to keep all individuals off you property

• Private Property - No Trespassing Sign Print out this sign to post on your property. Please note that a "No Trespass" letter must also be on file with us.

• Vacation Watch Form Complete this form and return it to the Community Service Bureau, via U.S. mail, e-mail, fax or just drop it off at our front counter. Your beat officer will be notified and extra patrols will be done as time permits.
